Acknowledgment
I understand that participating in exercise and using ES Gym facilities, equipment, classes,
recovery amenities, locker rooms, parking areas, and common areas involves inherent risks
including serious injury, illness, disability, or death. I voluntarily assume all such risks.
Release of Liability
To the fullest extent permitted by law, I release and hold harmless ES Gym, its owners, employees,
contractors, trainers, vendors, and affiliates from claims arising from my use of the facility or
participation in activities, except where prohibited by law.
Health Responsibility
I certify I am physically able to participate or have chosen to participate at my own risk. I am
responsible for obtaining medical clearance when appropriate and will stop exercising if I
experience concerning symptoms.
Recovery Amenities
I understand that infrared saunas, red light therapy, massage chairs, compression therapy, and
similar amenities carry risks and I use them voluntarily at my own risk.

Infrared Sauna & Red Light Therapy
- Use at your own risk.
- Consult your physician before use if you are pregnant, have a medical condition, or use a pacemaker or other implanted medical device.
- Do not use if you are taking medications that increase sensitivity to heat or light (photosensitive medications), unless approved by your healthcare provider.
- Do not use if you are ill, have a fever, open wounds, or are under the influence of alcohol or drugs.
- Stay hydrated before and after your session.
- Exit immediately if you experience dizziness, nausea, chest pain, difficulty breathing, skin irritation, or any discomfort.
- Follow all posted session time limits.
By using this equipment, you acknowledge and accept all risks associated with its use.
Kids Corner – Important Notice
ES Gym is NOT a daycare or supervised childcare service. Parents and legal
guardians retain full responsibility for supervising and ensuring the safety of their children at all
times. I accept all risks associated with my child's presence in ES Gym and release ES Gym
from liability for injuries or incidents to the fullest extent permitted by law.
Personal Property
ES Gym is not responsible for lost, stolen, or damaged personal property or vehicles.
Rules
I agree to follow all posted rules and staff instructions and understand that ES Gym may suspend or
terminate my membership for unsafe or inappropriate conduct.
Agreement
I have read and understand this waiver, voluntarily accept its terms, and understand that I am giving
up certain legal rights. You are allowing ES Gym staff and management to text and email your provided information on file.
